Background: Smoking is a significant health hazard and contributes to cardiovascular and pulmonary diseases. It can increase postoperative complications during oral and maxillofacial surgery due to its topical effect on the oral mucosa. New alternatives to traditional tobacco products are gaining popularity, in particular, electronic cigarettes. Objectives: This pilot study investigated the acute effects of nicotine-containing and nicotine-free electronic cigarettes on palatal blood flow (PBF), and compared their effects to traditional cigarettes. Materials and methods: 14 medically healthy volunteers (8 males, 6 females, age: 34.7 ± 7.0) were recruited for the study. All patients (N = 14) were requested to smoke nicotine-containing (ECN) and nicotine-free electronic cigarettes (EC0) and a mouthpiece (end of a traditional cigarette) as a control sham smoking blind test (BT). EC users did not smoke a traditional cigarette (TC), resulting in 10 people in the TC group. Palatal blood flow was measured by Laser Speckle Contrast Imager before, immediately after, and 15 min after the exposures. Exhaled carbon monoxide (eCO) and carboxyhemoglobin (COHb) were measured before and immediately after smoking with a piCO+ Smokerlyzer machine. Results: In all groups, no significant differences were observed in the changes of palatal blood flow between time points. Exhaled carbon monoxide and carboxyhemoglobin were significantly higher in the traditional cigarette (TC) group compared to the nicotine-containing electronic cigarette (ECN) and nicotine-free electronic cigarette (EC0) groups, both before and after the exposure (p < 0.05). Conclusion: Acute use of either traditional or electronic cigarettes may have minimal impact on palatal blood flow, but additional studies are required to clarify their impact on the mucosa.

OBJECTIVE: To evaluate the translucency of several monolithic zirconias (MZ) processed in various sintering ovens designed for single-visit, chairside use. METHODS: Discs (n = 40) from zirconia blocks were fabricated for each MZ at manufacturer-recommended minimal thicknesses, as provided in each material's instructions for use: IPS e.max ZirCAD LT (ZLT); CEREC Zirconia+ (CZ+); 3M Chairside (3M); KATANA Zirconia (KT). Groups (n = 10) were sintered following manufacturer instructions for each oven: CEREC SpeedFire, Ivoclar CS4, Ivoclar CS6, and Ivoclar S2 (laboratory furnace control). Specimens were highly polished on one side and glazed on the other. Each side was measured with a spectrophotometer against white and black backgrounds to determine translucency parameter (TP) and contrast ratio (CR) values. Results for TP and CR for each material and oven combination were compared with a linear mixed model. Oven precision was evaluated using the Kruskall-Wallis test. RESULTS: Glazed specimens were more translucent than polished ones (p < 0.001). ZLT and CZ+ were more translucent than 3M and KT regardless of the sintering oven (p < 0.01). Several oven/material combinations reached or exceeded the S2 oven TP: CS4 with CZ+ and 3M; CS6 with ZLT and KT (p < 0.01). SpeedFire was significantly less precise (p < 0.05) and produced lower TP values for ZLT, CZ+, and KT (p < 0.01). Results for TP and CR were highly correlated. CONCLUSIONS: MZ surface finish, material thickness, and oven used all had a significant effect on translucency. Some chairside-oriented solutions produced results with translucency equal to conventionally processed zirconia. CLINICAL SIGNIFICANCE: The translucency of a ceramic restoration is an important factor in determining its esthetics. Clinicians desiring the most esthetic outcomes with monolithic zirconia should be aware of the significant effects that surface finishing, material thickness, and the sintering oven used can have on restoration translucency.

OBJECTIVES: To validate the virtual-fit alignment, analyze the impact of cement spacing on internal/marginal gaps, and correlate results with conventional trueness measures. METHODS: Four dental abutment models were scanned using an industrial reference scanner (one time each), Emerald S (three times each), and Medit i700 (three times each) intraoral scanners (IOS). On each IOS scan (n = 24), three complete-arch fixed frameworks were designed with 70 or 140 µm cement space with no marginal space (groups 70 and 140) and 70 µm with an additional 20 µm space, including the margin (group 70+20). Two types of alignment were performed by GOM Inspect software. The reference and IOS scans were aligned through a conventional iterative closest point algorithm (ICP) where the penetration of the two scans was permitted into each other (conventional trueness method). Second, the computer-aided designs were superimposed with the reference scan also using an ICP, but preventing the design from virtual penetration into the model (virtual-fit method). The virtual-fit algorithm was validated by non-penetration alignment of the designs with the IOS scans. Internal and marginal gap was measured between the design and the abutments. The difference between spacing groups was compared by Friedman's test. A statistical correlation (Spearman's Rho Test) was computed between the measured gaps and the conventional trueness method. A significant difference was accepted at p<0.05 after the Bonferroni correction. RESULTS: The gaps deviated from the set cement space by 3-13 µm on IOS scans (validation of virtual-fit algorithm). The internal gap of the design on the reference scan was not affected by cement spacing (Emerald S, p = 0.779; Medit i700, p = 0.205). The marginal gap in groups 70 and 70+20 was significantly lower than in group 140 in Emerald S (p<0.05). In Medit i700, it was lower in the 70+20 group than in the group 70 (p<0.01) and in the group 140 (p<0.05). Some Medit i700 scans exhibited high marginal gaps within group 70 but not in groups 70 and 140. The measured gaps correlated significantly (r = 0.51-0.81, p<0.05-0.001) with the conventional trueness but were 2.6-4.6 times higher (p<0.001). CONCLUSION: Virtual-fit alignment can simulate restoration seating. A 20 µm marginal and 90 µm internal spacing could compensate for scan errors up to several hundred micrometers. However, 140 µm internal spacing is counterproductive. The conventional trueness method could only partially predict framework misfit. CLINICAL SIGNIFICANCE: The virtual-fit method can provide clinically interpretable data for intraoral scanners. Emerald S and Medit i700 intraoral scanners are suitable for fabricating complete-arch fixed tooth-supported prostheses. In addition, a slight elevation of spacing at the margin could compensate for moderate inaccuracies in a scan.

OBJECTIVES: The study aimed to compare the antibacterial effect of a novel disinfectant, hyper-pure chlorine dioxide (hClO2) to sodium hypochlorite (NaOCl) in various depths of dentin tubules. MATERIALS AND METHODS: The distal root of the extracted lower molars was infected artificially with Enterococcus faecalis. The control group was rinsed with saline, and the test groups were irrigated with either 5% NaOCl or 0.12% hClO2. The longitudinally split teeth were stained by viability stain. The coronal third of the root was scanned with a confocal laser scanning microscope. The fluorescent intensities were measured, and the percentage of dead bacteria was calculated at depths up to 950 µm along the dentin tubules. The effect of penetration depth, irrigants, and their interaction on antimicrobial efficacy was determined by the linear mixed model. RESULTS: The percentage of dead bacteria was higher both in the NaOCl (45.1 ± 2.3%, p < 0.01) and in the hClO2 (44.6 ± 3.8%, p < 0.01) irrigant groups compared to saline (23 ± 4.5%); however, there was no difference between them. The percentage of killed bacteria was not correlated with the depths in any group (p = 0.633). CONCLUSIONS: Our results suggest that the functional penetration depth of NaOCl is at least 2-3 times more than published to date. There is no difference in disinfection effectiveness along the dentin tubules between NaOCl and hClO2 until at least the measured 950 µm. However, both were only able to eradicate the intratubular bacteria partially. CLINICAL RELEVANCE: Hyper-pure ClO2 could be used as an alternative or final adjuvant irrigant in endodontic treatment.

OBJECTIVES: The study aimed to compare the trueness and precision of five intraoral scanners (Emerald S, iTero Element 5D, Medit i700, Primescan, and Trios 4) and two indirect digitization techniques for both teeth and soft tissues on fresh mandibular and maxillary cadaver jaws. METHODS: The maxilla and mandible of a fully dentate cadaver were scanned by the ATOS industrial scanner to create a master model. Then, the specimens were scanned eight times by each intraoral scanner (IOS). In addition, 8 polyvinylsiloxane (PVS) impressions were made and digitized with a Medit T710 desktop scanner. Stone models were then poured and again scanned with the desktop scanner. All IOS, PVS, and stone models were compared to the master model to calculate the mean absolute surface deviation for mandibular teeth, maxillary teeth, and palate. RESULTS: For mandibular teeth, the PVS trueness was only significantly better than the Medit i700 (p < 0.001) and Primescan (p < 0.05). In maxillary teeth, the PVS trueness was significantly better than all IOSs (p < 0.05-0.001); the stone trueness was significantly better than Emerald S (p < 0.01), Medit i700 (p < 0.001) and Primescan (p < 0.01). In the palate, PVS and stone trueness were significantly lower than the iTero Element 5D (p < 0.01) and Trios 4 (p < p < 0.01). Stone trueness was significantly lower than the Medit i700 (p < 0.05). The precision in the palate was significantly lower for PVS and stone than for Emerald S (p < 0.01, p < 0.05), iTero Element 5D (p < 0.01, p < 0.01), Primescan (p < 0.001, p < 0.001), and Trios 4 (p < 0.001, p < 0.01). Significant differences in trueness between the IOSs were observed only in the mandibular teeth. The Medit i700 performed worse than Emerald S (p < 0.01) and iTero Element 5D (p < 0.01). For mandibular teeth, the Medit i700 was significantly more precise than Primescan (p < 0.01) and the Emerald S (p < 0.05). The Trios 4 was significantly less precise than Emerald S (p < 0.05). The precision of Medit i700 was significantly worse than iTero Element 5D (p < 0.01) for maxillary teeth, as well as the Primescan (p < 0.01) and Trios 4 (p < 0.05) for the palate. CONCLUSIONS: In general, indirectly digitized models from PVS impressions had higher trueness than IOS for maxillary teeth; precision between the two methods was similar. IOS was more accurate for palatal tissues. The differences in trueness and precision for mandibular teeth between the various techniques were negligible. CLINICAL SIGNIFICANCE: All investigated IOSs and indirect digitization could be used for complete arch scanning in mandibular and maxillary dentate arches. However, direct optical digitization is preferable for the palate due to the low accuracy of physical impression techniques for soft tissues.

OBJECTIVES: As jaw-tracking systems integrate into digital prosthetic workflows, their accuracy remains underexplored. This study aimed to evaluate the in vitro accuracy of a novel digital jaw-tracking system (Modjaw, Villeurbanne, France) by comparing its precision and trueness to that of an industrial scanner. METHODS: Upper and lower typodont models were scanned with an industrial-grade optical scanner (ATOS Q, Carl Zeiss GOM Metrology GmbH, Germany) to produce master scans. The models were placed in a phantom head with artificial joints to replicate five different intermaxillary relationships (IMRs). The 1, 2, 3, 4, and 5 mm IMR distances were stabilized by five silicone bites. The silicone bites were repositioned after each measurement. ATOS scanned the whole artificial joint with the models three times in each IMR to assess the precision of the repositioning (i.e., bite precision). The master scans were uploaded to Modjaw. Modjaw recorded the five IMR positions three times each to assess the precision of the Modjaw. Precision was calculated by aligning the scans within the same group, whereas Modjaw trueness was evaluated by aligning ATOS and Modjaw scans. The mean absolute distance (MAD) between aligned surfaces was calculated. The effect of IMR on the MAD was evaluated using a linear mixed model. RESULTS: The mean bite precision across the IMRs was 7.6 ± 0.53 µm. Modjaw precision over the IMRS was 9.7 ± 1.76 µm, and the trueness was 10.8 ± 1.40 µm. Increased IMRs up to 4 mm significantly increased the MAD from 6.5 to 8.5 µm for the bite precision, 4.8 to 15.7 µm Modjaw precision, and 7.1 to 14.9 µm for trueness. CONCLUSIONS: Modjaw excelled in accuracy, comparable to industrial scanners and superior to traditional methods. IMR elevation marginally deteriorates the accuracy. Future studies should extend to varied movements beyond centric relations and encompass the influence of intraoral scanners.

OBJECTIVES: To investigate the intraosseous arterial pathways and anastomoses in the alveolar aspects of the maxilla in order to better understand the arterial scattering pattern. MATERIALS AND METHODS: Eleven cadavers were selected for macroscopic intraosseous arterial analyses by corrosion casting. The red-colored acrylic resin was injected into the external carotid arteries. The specimens were kept in an enzymatic solution at 36 °C for about 60 days, depending on the process progression. After removal of the soft tissues and drying, the bone was macerated by potassium hydroxide to analyze the course and the mean diameters of the intraosseous anastomoses. RESULTS: Vertico-oblique and horizontal intraosseous arteries and anastomoses between the greater palatine-, posterior superior alveolar-, and infraorbital arteries were detected. The vertico-oblique anastomoses were found on the anterolateral wall of the maxilla and the alveolar crest with a mean diameter of 0.46 mm; nevertheless, the horizontal (transalveolar) anastomoses were identified in the interdental septum/alveolar crest with the mean diameter of 0.41 mm. From the horizontal anastomoses, small intraseptal branches supplied the territory of the alveolar socket in various directions. CONCLUSIONS: The localization of intraosseous arterial anastomoses is critical in implant-related surgeries, predominantly to maintain proper circulation. CLINICAL RELEVANCE: Based on vertico-oblique and transalveolar anastomoses, simultaneous buccal- and palatal flap elevation (particularly on the palatal side) should be avoided to minimize patient morbidity and intra- or postoperative complications. Moreover, preserving transverse loops in the interdental septum is essential during implant surgeries, which can significantly influence collateral periosteal and osteal circulation to prevent ischemia.

BACKGROUND: Symmetry is critical in perceived attractiveness, especially in female faces. The palate determines the teeth' alignment and supports facial soft tissues. Therefore, the study aimed to assess the effects of sex, orthodontic treatment, age, and heritability on the directional, anti-, and fluctuational asymmetry in the digital palatal model. METHODS: The palate of 113 twins, 86 female and 27 male subjects, with and without previous orthodontic treatment, were scanned by the Emerald (Planmeca) intraoral scanner. Three lines were constructed horizontally in the digital model, one between the right and left first upper molars and two between the first molars and incisive papilla. Two observers calculated the left and right angles between the mid-sagittal plane and molar-papilla lines. The intraclass correlation coefficient was used to assess the inter-observer absolute agreement. The directional symmetry was determined by comparing the mean left and right angles. The antisymmetry was estimated from the distribution curve of the signed side difference. The fluctuating asymmetry was approximated from the magnitude of the absolute side difference. Finally, the genetic background was assessed by correlating the absolute side difference between monozygotic twin siblings. RESULTS: The right angle (31.1 degrees) was not significantly different from the left one (31.6 degrees). The signed side difference followed a normal distribution with a mean of -0.48 degrees. The absolute side difference (2.29 degrees, p < 0.001) was significantly different from zero and negatively correlated (r=-0.46, p < 0.05) between siblings. None of the asymmetries was affected by sex, orthodontic treatment or age. CONCLUSIONS: The palate illustrates neither directional asymmetry nor antisymmetry, indicating that most people's palates are symmetric. However, the significant fluctuating asymmetry suggests that some subject has considerable asymmetry but is not influenced by sex, orthodontic treatment, age, and genetics. The proposed digital method is a reliable and non-invasive tool that could facilitate achieving a more symmetrical structure during orthodontic and aesthetic rehabilitation. OBJECTIVES: The palatal scans of the same individuals were compared after two years to assess forensic reproducibility. The effect of orthodontic treatment, the comparison area and the digitization approach were investigated. METHODS: The palate was scanned three times in 20 pairs of monozygotic twins with an intraoral scanner (IOS), to assess repeatability. The same subjects were re-scanned two years later, with two different IOSs. An elastic impression and a plaster model were also made and scanned with a laboratory scanner (indirect digitization). Mean absolute distance between scans was compared after best-fit alignment. Scans from the two sessions were compared to evaluate the combined effect of aging, orthodontic treatment and different digitization methods (forensic reproducibility). Additionally, the scans of different digitization methods from the second session were compared (technical reproducibility). The between-siblings difference was compared in the two sessions, to evaluate the effect of aging on palatal morphology. RESULTS: The anterior palatal area showed significantly better repeatability and forensic reproducibility than the whole palate (p < 0.001), but orthodontic treatment had no effect. Indirect digitization produced lower forensic and technical reproducibility than IOSs. For IOSs, repeatability (22 µm) was significantly (p < 0.001) better than either forensic (75-77 µm) or technical reproducibility (37 µm). No significant changes were observed from the first to the second session in the between-sibling comparison. The closest between-sibling value (239 µm) considerably exceeded the highest forensic reproducibility value (141 µm). CONCLUSIONS: Reproducibility is acceptable between the different IOSs, even after two years, but is poor between IOS and indirect digitization. The anterior palate is relatively stable in young adults. CLINICAL SIGNIFICANCE: Intraoral scanning of the anterior palatal area has superior reproducibility, regardless of the IOS brand. Therefore, the IOS method could be suitable for identifying humans through anterior palatal morphology. However, the digitization of elastic impressions or plaster models had low reproducibility, preventing their application for forensic purposes.

Our research group developed a novel nano-pitted (NP) TiO2 surface on grade 2 titanium that showed good mechanical, osteogenic, and antibacterial properties; however, it showed weak hydrophilicity. Our objective was to develop a surface treatment method to enhance the hydrophilicity of the NP TiO2 surface without the destruction of the nano-topography. The effects of dilute and concentrated orthophosphoric (H3PO4) and nitric acids were investigated on wettability using contact angle measurement. Optical profilometry and atomic force microscopy were used for surface roughness measurement. The chemical composition of the TiO2 surface and the oxidation state of Ti was investigated using X-ray photoelectron spectroscopy. The ccH3PO4 treatment significantly increased the wettability of the NP TiO2 surfaces (30°) compared to the untreated control (88°). The quantity of the absorbed phosphorus significantly increased following ccH3PO4 treatment compared to the control and caused the oxidation state of titanium to decrease (Ti4+ → Ti3+). Owing to its simplicity and robustness the presented surface treatment method may be utilized in the industrial-scale manufacturing of titanium implants.

OBJECTIVES: The study aimed to compare the precision of ATOS industrial, 3ShapeE4, MeditT710, CeramillMap400, CSNeo, PlanScanLab desktop, and Mediti700 intraoral scanners. The second aim was to compare the trueness of Mediti700 assessed by ATOS and desktop scanners. METHODS: Four plastic dentate models with 7-12 abutments prepared for complete arch fixed dentures were scanned by all scanners three times. Scans were segmented to retain only the abutments. The precision and trueness were calculated by superimposing scans with the best-fit algorithm. The mean absolute distance was calculated between the scan surfaces. The precision was calculated based on the 12 repeats. Trueness was evaluated by superimposing the desktop and IOS scans to the industrial scans. IOS was also aligned with the two most accurate desktop scanners. RESULTS: The precision of 3ShapeE4 and MeditT710 (3-4µm) was only slightly lower than that of ATOS (1.7µm, p<0.001) and significantly higher than CeramillMap400, CSNeo, and PlanScanLab (6-10 µm, p<0.001). The trueness was the highest for the 3Shape E4 (12-13 µm) and Medit T710 (13-16 µm) without significant difference. They were significantly better than CeramillMap400, CSNeo, and PlanScanLab (22-31µm, p<0.001). Accordingly, the Mediti700 trueness was evaluated by ATOS, 3ShapeE4, and MeditT710. The three trueness was not significantly different; ATOS (23-26 µm), 3Shape E4 (22-25 µm), and Medit T710 (20-23 µm). CONCLUSIONS: All desktop scanners had the acceptable accuracy required for a complete arch-fixed prosthesis. The 3Shape E4 and the Medit T710 might be used as reference scanners for studying IOS accuracy. CLINICAL SIGNIFICANCE: 3ShapeE4, MeditT710, CeramillMap400, CSNeo, PlanScanLab laboratory, and Mediti700 intraoral scanners can be used for the prosthetic workflow in a complete arch. 3ShapeE4 and the MeditT710 could be used to test the accuracy of various phases of a laboratory workflow, replacing the industrial scanners.

Previous 3D superimposition studies of digital scans of the human palate, using geometric and surface morphology comparisons, have shown its usefulness in assisting in the identification process, including its ability to distinguish identical twins. This study aimed to evaluate the discriminative potential when only simple geometric analysis is used. Its aim is not only to determine if geometric comparison alone is sufficient not only to assist in identification but if it supports the hypothesis of assisting in sex discrimination when no other comparative data is available. The palates of 64 monozygotic (M.Z.T.) and 39 dizygotic (D.Z.T.) twins were digitized three times using a scanner. Digital smoothing was used to remove the rugae, and palatal height, depth, and width were measured. The study confirmed that the smoothing function had little effect on the discriminatory function since the Mean Absolute Distance (MAD) between M.Z.T. (0.430 ± 0.018 mm versus 0.425 ± 0.022 mm p = 0.061) or D.Z.T. (0.621 ± 0.058 mm versus 0.586 ± 0.053 mm, p = 0.284) scans show little change. By combining the height, depth, and width into a discriminative function, the sex correctly correlated 83.9% of the time, identity by 91.2% sensitivity, and twining by 68.5%. The difference in the 3D palatal model between twin siblings is primarily due to palate geometrics. Since geometric comparison requires far less computation time, geometric comparison alone can be used as an adjunct metric for limiting the possible matches in a dental 3D database in determining both sex and identity, especially if no other evidence is available.

BACKGROUND: Sex hormones influence circulation, periodontitis, and wound healing. The aim of the study was to compare the endothelium-dependent and independent vasodilation in human gingiva in men and women. METHODS: Gingival blood flow was evaluated in twelve male and twelve female subjects with healthy gingiva and no systemic conditions after acetylcholine or nitric oxide donor (NitroPOHL). Agonists were administered into the gingival sulcus at the right secondary incisor (test site). Regional gingival blood flow (GBF) was imaged by Laser Speckle Contrast Imager from the marginal gingiva to the mucogingival junction in four consecutive regions (coronal, midway1, midway2 and apical). Blood flow was expressed in Laser Speckle Perfusion Unit (LSPU). The absolute maximal blood flow change (Dmax), the area under the blood flow curve (AUC), and the time to peak (TTP) were calculated. RESULTS: Males had higher baseline GBF than females (257 ± 18.2 vs. 225 ± 18.8 LSPU, p < 0.001). Acetylcholine and NitroPOHL significantly increased the GBF in all test regions. The Dmax after the acetylcholine was reduced apically compared to the coronal (90 ± 13 LSPU vs. 117 ± 7 LSPU, p < 0.01), but it was similar after NitroPOHL (78 ± 9 LSPU vs. 86 ± 6 LSPU, p = 0.398) in both sexes. The Dmax and AUC were higher, and the TTP was smaller in men in most regions after acetylcholine but not after NitroPOHL. CONCLUSION: In the human gingiva, the endothelium-independent vasodilation propagates without attenuation in the line of the vascular supply in both sexes. At the same time, the endothelium-dependent ascending vasodilation attenuates similarly in men and women. However, men had more pronounced endothelium-dependent vasodilation than women. Therefore, it might contribute to the increased severity of periodontal disease in men. TRIAL REGISTRATION: The study was registered with ClinicalTrials.gov on 09.06.2021 (NCT04918563).

STATEMENT OF PROBLEM: A custom emergence profile offers the ideal horizontal dimensions for an anatomic healing abutment. However, developing such an emergence profile can be a time-consuming and complex process. PURPOSE: The purpose of this study was to develop a mathematical formula defining horizontal cervical tooth geometry to design prefabricated, tooth-specific, healing abutments. MATERIAL AND METHODS: Cone beam computed tomography (CBCT) horizontal cross sections of 989 teeth on 54 participants were measured. For anterior and premolar teeth, 2 perpendicular ellipses were fitted onto the cervical tooth cross section that was defined by 3 parameters. The lingual ellipse followed the lingual outline of the tooth, and its diameter was the largest mesiodistal diameter of the tooth (parameter "a"); its buccolingual radius became parameter "b." The buccal ellipse was perpendicular to the lingual ellipse and followed the buccal outline of the tooth. The buccolingual radius of the smaller ellipse became parameter "c." For molars, the first ellipses followed the mesial outline of the tooth, and its larger diameter (parameter "a") matched the largest buccolingual diameter of the tooth. Its smaller radius became parameter "h1." The second ellipse was parallel to the first ellipse and followed the distal outline of the tooth. Its larger diameter became parameter "b", and its mesiodistal diameter became parameter "h2". Statistical differences between parameters were evaluated by the linear mixed model (α=.05 after Bonferroni adjustment). Pairwise comparisons were made separately for each parameter of the molars and separately for each parameter for the anterior teeth plus premolars. Teeth were put into the same parameter cluster if no significant differences were found between them for a specific parameter. If neither parameter (4 for molars and 3 for the other teeth) was different for 2 teeth, they were put into the same abutment cluster. The abutment clusters determined the type of anatomic healing abutment. The areas were calculated from the developed mathematical formula by using the parameters. In addition, cervical areas of 106 randomly chosen teeth were measured directly with a photo-editing software program. A computer algorithm was used to select 5 CBCT scans from the 54 by using the simple randomization method. The agreement between the 2 methods was evaluated by Bland-Altman analysis. RESULTS: The lower and upper limits of agreement between the 2 methods were -8.57 and 7.36 mm2, respectively, with no bias (-0.61 mm2, P=.224). Significant differences were found between most parameters among the 14 tooth types (P<.001). Based on the parameters, 12 specifically distinct clusters were defined. Two tooth types were pooled into 1 abutment cluster: the maxillary first and second premolars and the mandibular first and second molars. CONCLUSIONS: The cervical tooth cross section can be accurately defined by combining 2 elliptical elements. A comprehensive array of tooth specific emergence profiles can be provided by just 12 different prefabricated abutments, designed as per the recommended parameters.

STATEMENT OF PROBLEM: The intraoral scanning of the edentulous arch might be challenging for an inexperienced operator because of the large mucosal area and the use of scan bodies. PURPOSE: The purpose of this ex vivo study was to compare the trueness of 5 intraoral scanners in replicating implant scan bodies and soft tissues in an edentulous maxilla and to investigate the effects of operator experience. MATERIAL AND METHODS: The maxilla was resected from a fresh cadaver, 5 implants placed, and a reference scan made. Eight scans were made by experienced operators and 8 by an inexperienced operator with each scanner (iTero Element 2, Medit i500, Primescan, TRIOS 3, TRIOS 4). The implant platform deviation was measured after complete surface alignment and after scan body alignment. Deviation data were analyzed with a generalized linear mixed model (α=.05). RESULTS: After complete surface alignment, the mean ±standard deviation implant platform deviation was higher for the inexperienced operator (421 ±25 µm) than for experienced ones (191 ±12 µm, P<.001) for all scanners. After scan body alignment, no significant differences were found between operators for Element 2, Primescan, and TRIOS 3. The experienced operators produced a lower deviation for TRIOS 4 (35 ±3.3 µm versus 54 ±3.1 µm, P<.001), but higher deviation for i500 (68 ±4.1 µm versus 57 ±3.6 µm, P<.05). The scanner ranking was Element 2 (63 ±4.1 µm), i500 (57 ±3.6 µm, P=.443), TRIOS 4 (54 ±3.1 µm, P=.591), TRIOS 3 (40 ±3.1 µm, P<.01), Primescan (27 ±1.6 µm, P<.001) for the inexperienced operator and i500 (68 ±4.1 µm), Element 2 (58 ±4.0 µm, P=.141), TRIOS 3 (41 ±2.8 µm, P<.001), TRIOS 4 (35 ±3.3 µm, P=.205), Primescan (28 ±1.8 µm, P=.141) for the experienced operators. CONCLUSIONS: Mucosal alignment greatly overestimated the platform deviation. The intraoral scanners showed different trueness during the complete-arch implant scanning. The operator experience improved the trueness of the edentulous mucosa but not implant platform deviation.

OBJECTIVES: We aimed to study whether or not various dental antiseptic agents affect the viability and proliferation of human periodontal ligament cells (PDLCs). MATERIALS AND METHODS: Human PDLCs were isolated from a total of 10 surgically extracted impacted third molars and were.cultured in-vitro. The cells were exposed to commonly used dental antiseptics, including chlorhexidine, cetylpyridinium chloride, triclosan, povidone-iodine and sodium bicarbonate for ultra-short-term (10, 20, 30 sec), short-term (10, 20, 30 min) and long-term (24, 48 h) at various concentrations. Cell morphology was observed with light microscopy. Cell viability was studied with impedimetric real-time xCELLigence and resazurin-based alamarBlue® assays. We used one-way ANOVA with Tukey's and Bonferroni test (p < 0.05) for statistical analysis. RESULTS: Both alamarBlue® and xCELLigence analysis results were in agreement that ultra-short-term contact with cetylpyridinium chloride ≥ 0.01 mg/ml, chlorhexidine ≥ 1 mg/ml, triclosan ≥ 1 mg/ml and povidone-iodine ≥ 1 mg/ml as well as long-term exposure to cetylpyridinium chloride ≥ 0.001 mg/ml, chlorhexidine ≥ 0.01 mg/ml, triclosan ≥ 1 mg/ml, povidone-iodine ≥ 1 mg/ml and sodium bicarbonate ≥ 10 mg/ml was able to reduce the viability of human PDLCs significantly. According to the half-maximal inhibitory concentration (IC50) the rank of cytotoxicity was cetylpyridinium chloride > chlorhexidine > triclosan > povidone-iodine > sodium bicarbonate. CONCLUSIONS: Our findings suggest that the tested antiseptic agents were cytotoxic to human PDLCs at lower than practically applied concentrations in dental interventions.

The morphology and histology of the soft tissue around the implant are different from the periodontal tissue, but the difference in the regulation of blood flow is not known. The aim of the study was to compare the resting blood flow and the vasodilatation capacity of the gingiva between implants and teeth. Twenty-six healthy volunteers with single-tooth implants were involved. The implant-borne crown was retained on either a zirconia or titanium abutment. The vasodilatation capacity of the gingiva was assessed by a postocclusive reactive hyperemia test. Blood flow was measured by a laser speckle contrast imager at the buccal gingiva of the implant-borne crown and an analog natural tooth. No significant differences in baseline gingival blood flow were observed between the different abutments and the teeth in either region. The hyperemia after compression was significantly attenuated at the zirconia abutments in all regions during the entire investigation period (20 minutes) compared to the titanium abutments and the teeth. No differences were observed between titanium abutments and the teeth. The resting microcirculation seems to be the same at implants and teeth. However, the vascular reactivity might be disturbed at the zirconia, but not at the titanium, abutment.

OBJECTIVES: The goal of the study was to determine the effects of software updates on the trueness and precision of digital impressions obtained with a variety of intraoral scanner (IOS) systems. METHOD AND MATERIALS: Seven IOS systems were investigated. Each system was tested using two versions of software, with the second version being the latest at the time of conducting the study. Scans were performed on a custom mandibular typodont model with natural teeth that were either unrestored or restored with amalgam, composite, lithium disilicate, zirconia, and gold. Eight scans were obtained for each software version on any of the tested IOS systems. Experimental IOS scans were compared against an industry-standard master scan of the typodont obtained with an ATOS Capsule scanner proven to have a trueness of 3 µm and a precision of 2 µm. Isolation of each substrate material on the digital experimental and master scans was achieved using the Geomagic metrology software for subsequent analysis of the substrate influence on accuracy. A generalized linear mixed model was used to determine the influence of the software version on the trueness and precision of the impression scan. RESULTS: For some IOS systems, scans made with older software versions differ in accuracy compared with those obtained with the most recent software versions. Trueness was improved for most scanners following the software update, although the Element2 IOS performance deteriorated. Software updates had lesser effects on precision and showed variable trends among different systems. Software updates also influence different substrate materials scans' accuracy, although the results show variability among IOS systems. When comparisons were done among IOS systems updated with the latest software version, best performers for complete arch trueness were the Emerald S, Trios 3, and Primescan systems. CONCLUSION: Software updates have a statistically significant effect on the trueness and precision of different IOS systems. These updates can have both positive and negative effects on scan accuracy, although it appears that these variations are within the clinical acceptability levels.

BACKGROUND: The active arterial-to-arterial collaterals are a significant factor in the prevention of ischemia and extensive tissue necrosis in the case of arterial blockage of various tissues. The present study investigates the mucogingival vasculature in the maxillary esthetic zone mucosa in human cadavers and functionally evaluates the area, which is supplied by the terminal arterioles, on the individual level. METHODS: In the human cadaver study, macroscopic arterial analyses of the anterior maxillary vestibule in 7 specimens were scrutinized by latex milk injection. The tracks of the mucosal branches in relation to the mucogingival junction were investigated. In the functional study, individual gingival blood flow (GBF) changes were measured by laser speckle contrast imaging (LSCI) in 31 young subjects with healthy gingiva before and during 30-s compressions. This was conducted with a ball-shaped condenser. The data was analyzed by the linear mixed model. RESULTS: The vertically aligned branches of the superior labial artery (SLA) divided into small, slightly deviating sub-branches near the mucogingival junction. These arteries created collateral plexuses and supplied the attached gingiva. The compression of these branches resulted in ischemia coronally with significant individual variation. The ischemia was either apico-mesial, apico-distal, or straight apical to the compression. A significant correlation was found between the ischemic area and the magnitude of the decrease in GBF (r = 0.81, p < 0.001). In males, 77% of the subjects, and 50% of the female subjects had an ischemic response in either region. The horizontal extension of the ischemic area ranged between 0.26 mm and 8.76 mm. Males had significantly higher baseline GBF and larger ischemia than females. At the base of the papilla, significant restoration of GBF was observed during compression in males, but not in females. CONCLUSION: The arcade anastomoses formed by the small arteries in the keratinized gingiva of the upper esthetic zone explain the consequences of vertical incisions. The considerable individual variations in ischemic responses might be the reason for unexpected surgical outcomes in some cases. Furthermore, there is increasing evidence that men have different vascular reactivity and/or regulation of collateral circulation than women, which may affect wound healing.
